With this Privacy Policy,  Reedruus OÜ/City Tervisekliinik (registry code 11192652) (hereinafter referred to as “CTK”) gives an overview of the personal data collected by them which are necessary for providing the service.

1.1. Personal data means any information about a natural person, such as the person’s name, address, contact details, etc. Personal data also includes special categories of personal data, such as data concerning health.

1.2. Processing of personal data means any operation performed with personal data. CTK processes personal data, including data concerning health, only to the extent necessary for the performance of the tasks assigned to it and for providing swift and high-quality service to individuals. CTK has a legal obligation to protect the privacy of its clients and users.

1.3. We shall collect and process your personal data only if there is a legal basis for doing so and only for legitimate purposes.

1.4. We shall collect and process your personal data if:

• you come to our clinic – we shall process your personal data, including data about any medical conditions, when diagnosing and treating diseases;
• the patient has indicated you as their contact person – we shall process your personal data to provide you with information about the patient;
• you want us to issue your medical files – we shall use your personal data or the personal data of the person requesting the documents with your consent, to issue the documents;
• you send us a request for clarification, a note, a request for information or a complaint – we shall use your personal data to investigate the circumstances of the complaint and respond to you;
• you submit to us a proposal or a letter of appreciation – we shall publish your name on the clinic’s website and Facebook page with your consent;
• you are applying for a job with us – we shall rely on the information you provided. Every candidate has the right to know what data we have collected about them.

2.1. CTK shall issue data relating to you to the authorities or to persons who are legally entitled to request such data (e.g. the Estonian Health Insurance Fund, the Health Board, the insurer in the event of an insured event, the police, etc.). Documents with special categories of personal data are sent to recipients by registered mail or in an encrypted form via e-mail.

3.1. CTK has introduced the necessary information and cyber security measures and stores patients’ personal data in a manner that ensures the efficient protection of personal data in conformity with the law.

3.2. CTK shall make every effort to protect your personal data and to comply with data protection and privacy laws. Only persons who need it to perform their work duties shall be allowed to access the patients’ personal data. Our employees are subject to an obligation of confidentiality and we shall make every effort to protect your personal data which we hold.

4.1. You have the right to access the data we have collected. You can send your request at City Health Clinic or by e-mailing info@citytervisekliinik.ee.

4.2. Data are issued on the basis of an identity document or e-mailed in an encrypted form.

4.3. You have the right demand the correction or deletion of incorrect personal data.

5.3. I confirm that I have examined this Privacy Policy and fully understand and expressly consent to CTK collecting, using and transferring my personal data in accordance with the terms and conditions provided in this Privacy Policy.
